Go here.
Install Windows version via Crossover/Wine so it may be used with VirtualDub or other Windows programs you may have.
Copy the UT Video QuickTime Component to /Library/QuickTime for all users, and ~/Library/QuickTime just for your current user.
I have tested encoding video in UT Video in VirtualDub via Crossover, using MPEGStreamClip to change the container from .AVI to .MOV and placing the .MOV file into Premiere where it runs perfectly.
This topic will eventually be merged with a new guide.

Re: Install UT Video Now
UTV for Mac will encode to AVI container soon as well. 64bit ASM still an issue though. Related, libav has a fully functional UTV decoder, and ffmpeg has a buggy encoder as well. Still, building x264 against a recent version of libav (via homebrew) results in x264 reading UTV natively, which is really nice for quick final encode quality checks and whatnot.
